• Yearly Dog Grooming Policies and Release of Liability

    THIS AGREEMENT is entered into by and between Paw Purs Place at Mile Creek and Dog/Cat Owner
  • 1. OWNER RESPONSIBILITY
    I (Owner) represent that I am the legal owner of the above named animals and I assume all risks, dangers, and responsibility for injuries to the above named animals. Owner understands and agrees that Owner is solely responsible for any harm while Owner's pet(s) is/are attending Daycare, Boarding, and Grooming.

    2. PHOTO AND VIDEO RELEASE
    We love to post pictures and videos on Facebook, our website, and/or in ads. Owner agrees to allow Paw Pur’s Place at Mile Creek to use Owner’s pet’s name and any images or likeness of Owner's pet taken while he or she is at Paw Pur’s Place in any form, for use at any time, in any media, marketing, advertising, illustration, trade, or promotional materials without compensation. Owner releases to Paw Pur’s Place all rights that Owner may possess or claim to such image, likeness, recording, etc.

    3. DOG AND CAT HEALTH REQUIREMENTS
    Owner understands and agrees that Owner's dogs and or cats are healthy and will at all times while attending Paw Pur’s Place have current vaccinations against: Canine: Rabies, Distemper, Parvo, Adenovirus, Parainfluenza, Canine Influenza, and Bordetella Feline: FELV, Rabies, and FRCP Services also require annual heartworm testing for dogs. Cats are required to have tested negative for FELV and FIV. Negative annual fecal tests are required for both species. Dogs are required to be on a heartworm preventative with proof of purchase provided. Owner is not enrolling any pet that has any condition that could potentially jeopardize the health of other pets or people and has not had any potentially communicable condition within 30 days prior to enrollment. Owner further understands that even if Owner's dog is vaccinated for Bordetella, there is still a chance that Owner's dog can contract kennel cough. Owner agrees that Owner will not hold Paw Pur’s Place responsible if Owner's dog contracts kennel cough or other dog to dog transmitted illnesses.

    4. FLEA AND TICK FREE POLICY
    All pets must be flea and tick free. Proof of current flea and tick medication application must be provided year round. Refusal or lack of proof will result in termination of services. Should a pet be observed to have fleas or ticks, a Capstar flea medication will be applied and charged to the Owner at a rate of up to $80.00 depending on severity and size of dog or cat. If Owner is a Mile Creek Animal Hospital client and no prevention is on file prior to services, a 30 day preventative will be administered and charged to Owner.

    5. GROUP PLAY POLICY
    All dogs must pass a general behavior assessment in order to attend dog to dog group play. No dog may be admitted to group play who shows any signs of aggression. Owner understands and agrees that by admitting Owner's dog to group play, Owner represents that Owner's dog is in good health and has not harmed, shown aggression, or exhibited threatening behavior toward any person or other dogs. Any dog demonstrating aggression or other behavior deemed unacceptable by Paw Pur’s Place staff may be required to leave daycare or boarding.

    6. RELEASE OF LIABILITY
    Owner understands and agrees that during normal dog play and grooming, Owner's pet may sustain injuries. Activities are monitored by Paw Pur’s Place staff to best avoid injury, but scratches, punctures, torn ligaments, and other injuries may occur despite the best supervision. Owner further understands and agrees that Paw Pur’s Place, its employees, staff, or volunteers shall not be liable for any illness, injury, death, and or escape of Owner's pet(s) provided that reasonable care and precautions are followed. Owner hereby releases Paw Pur’s Place staff and volunteers from any liability of any kind whatsoever arising from or as a result of Owner's pet(s) attending Paw Pur’s Place at Mile Creek.

    7. PERSONAL PROPERTY AND DAMAGES
    Owner understands that Owner is solely responsible for any harm including harm to other pets, employees, invitees, equipment, facilities, or other property of Paw Pur’s Place caused by Owner's pet(s). Owner also agrees that Paw Pur’s Place shall not be responsible or liable for any lost or damaged personal property belonging either to Owner or Owner's pet.

    8. FOOD AND MEDICATIONS
    When boarding overnight, Owner agrees it is Owner's responsibility to leave an adequate supply of food and medications for Owner's pet(s) for the entire stay. Should food supply need replacement, Owner authorizes Paw Pur’s Place to use discretion in replacing and feeding appropriate food. All medications, vitamins, and supplements must be provided in original packaging in order to be administered by Paw Pur’s Place or Mile Creek Animal Hospital staff.

    9. VETERINARY LIABILITY AND CARE
    Owner agrees to allow Paw Pur’s Place to obtain medical treatment for Owner's pet(s) if they appear ill, injured, or exhibit behavior that reasonably suggests medical care is necessary, including anesthesia. Owner agrees to be fully responsible for the cost of any such medical treatment and gives permission for Paw Pur’s Place to use Mile Creek Animal Hospital for required treatment.
  • 10. WEAKENED IMMUNE SYSTEM
    Owner understands that special needs animals, young animals, and senior animals naturally have a higher risk of injury, stress related illnesses, weakened immune systems, or exacerbation of any pre existing conditions. As such, by using our facility for daycare or boarding, Owner is waiving any claim for injury or illness experienced by Owner's dog(s) while in our care.

    11. ELDERLY ANIMALS
    Owners of elderly dog(s) approaching the end of life understand that in the event the dog(s) passes while in our care, our policy is to hold the animal until the Owner returns or until we are able to contact Owner regarding disposition of the animal.

    12. GROOMING
    Grooming appointments will be scheduled based on availability. Owner may drop off before the scheduled appointment time but will be charged an additional fee if dropped off earlier than one hour prior to the scheduled time and or picked up later than one hour after completion call. Grooming services may be requested but are not guaranteed while pets are in our care for boarding or daycare. Grooming services are not included in daycare or boarding costs. Grooming cost is based on the following factors: size of the pet, condition of the coat, and pet temperament.

    13. PAYMENTS
    All services are due and payable at the end of each visit. Payment must be made in United States currency, Visa, Discover, American Express, or MasterCard. A $25 NSF charge will be applied for returned checks. Credit card information is not kept on file unless a completed authorization form is returned. This form may be requested at any time from front desk staff.

    14. CANCELLATION AND NO SHOW POLICY
    Cancellations of any services must be made at least 48 hours prior to the reservation date. If cancellation notice is less than 48 hours, a late cancellation fee will be added to Owner's account. Failure to arrive for scheduled services without notice will result in a no show fee charged to Owner's account. If Owner arrives late for a scheduled grooming appointment, the groomer may intake the pet if time allows or reschedule the appointment for the next available date. Repeated offenses will require prepayment for future appointments.

    15. CLOSING TIME
    Owner agrees that if Owner's pet(s) are not picked up by the end of business hours, Owner authorizes overnight boarding as appropriate and agrees to pay Paw Pur’s Place all incurred charges.

    16. RIGHT OF REFUSAL
    Paw Pur’s Place at Mile Creek reserves the right to refuse admittance to any dog(s) or dismiss any dog that does not meet or maintain health, temperament, or daycare and boarding standards. Determination shall be made at the sole discretion of Paw Pur’s Place at Mile Creek.

    17. FULL FORCE AND EFFECT
    Owner understands and expressly agrees that each and every provision contained in paragraphs 1 through 16 shall remain in full force and effect and shall apply to every occasion in which Owner's pet(s) attend Paw Pur’s Place at Mile Creek for daycare, extended boarding, grooming, or other services. This Agreement shall remain in effect unless canceled or superseded by a written agreement signed by both parties.

  • Owner hereby certifies that Owner has read and understands this Waiver and Release of Liability and the regulations set forth above. By signing this agreement, Owner agrees to be bound by its terms and conditions.
